From the East: Mass Pike to Exit 11A (495 North) / 495 North to I-290 West (Worcester)
I-290 West to Exit 22, (Shrewsbury, Worcester). Right at the bottom of the ramp onto West Main St. Go .06 miles and turn right at the second set of lights where Plantation Street becomes N. E. Cutoff and then becomes Mountain Street East. Continue through a third set of lights at Clark Street. Turn into the entrance for Briarwood Continuing Care Retirement Community. Follow signs to Knollwood Nursing Center.
From the West: Mass Pike to Exit 10 (I290 East)
I-290 East to Exit 21, (Plantation Street). Left at the bottom of the ramp. Straight north through two sets of lights. Plantation Street becomes N. E. Cutoff and then becomes Mountain Street East. Continue through a third set of lights at Clark Street. Turn into the entrance for Briarwood Continuing Care Retirement Community. Follow signs.
From the South: 495 North to I-290 West

From the North: Route 2 or 495 South to I-190
I-290 South to Exit 3 (West Mountain Street, Holden). Left at the end of the ramp on Mountain Street West through the lights at the bottom of the hill. Continue up the next hill through the second set of lights straight across Route 12 onto Mountain Street East. Go Approximately 1-1/2 miles and turn left into the entrance for Briarwood Continuing Care Retirement Community. Follow signs to Knollwood Nursing Center.
